Consultancy Service: Alarm Rationalisation study

Case Study: The project scope was to carry out an assessment of a Top Tier COMAH facilities current alarm database in compliance with the good practice guidance provided by EEMUA 191. The process involved initial preparation of the customers alarm database and then use of a software based EEMUA 191 alarm rationalisation tool to document alarm details including purpose, required operator action and consequence of no action followed by classification of each alarm within a multidiscipline team review structure. 

 

Consultancy Service: Functional Safety Assessment

Case Study: An independent Functional Safety Assessment (FSA) was required on a metal dissolving facility Safety Instrumented System that included multiple Safety Instrumented Functions. The assessments were carried out after Safety Requirement Specification production, post Design & Engineering completion and prior to start up of the facility. This process was successfully integrated with the clients own Functional Safety Management system. The assessment was carried out to assure compliance with the requirements IEC61511:2004 and provided an independent judgement on the functional and safety integrity achieved. 

 

Consultancy Service: Management of Functional Safety

Plant: OIL and GAS 

Case Study: This assignment involved support of a top tier COMAH facility in implementing a functional safety management system in compliance with the requirements of IEC61511:2004. The initial project scope was to carry out a functional safety capability analysis of the facility management systems to identify deficiencies when considered against the requirements of IEC61511:2004 clause 5.

After reporting the gaps in the management system the second phase of the project involved development of procedures and documents to close the gaps and achieve compliance with the requirements of IEC 61511:2004. The functional safety management system was successfully implemented and continued support was provided as the system matured and developed.

 

Consultancy Service: Hardware Verification Calculation

Case Study: This assignment was successfully awarded and required the re-verification of the hardware PFDAVG for the Safety Instrumented Functions of the Gas Process Facility.  Further work was undertaken identifying, which safety instrumented functions could extended the required proof test interval to be inline with the facility maintenance shutdown, while operating within the organisations defined tolerability requirements of the IEC 61511 SIL ranges.

 

Consultancy Service: Independent Hardware Verification Calculation

Lifecycle Activity: Phase 9: Verification

Plant: Oil and Gas

Case Study: This assignment was successfully awarded and an independent verification of the calculated engineered safety instrumented function.  The previous verification calculation had used the monte-carlo methodology.  The clients’ third party verification body and the local regulator requested verification that the obtained results were not overtly optimistic. ProSalus independently verified the hardware using the event space method.  This assignment was completed and reviewed by the third-party verification body and the local regulatory body to all parties’ satisfaction.
