Plant: Karachaganak HIPPS Project
Case Study:
Independent Safety Assessment of a SIL3 High Integrity Pressure Protection System on an existing gas injection well. The project required the facilitation of the Safety Integrity Level (SIL) Determination study to identify the target SIL, assessment of the current design and providing recommendations on SIL improvement. The SIL verification of achieved probability of failure demand and spurious trip rate were determined using fault tree analysis.
Contract: Plant Safety Lifecycle (IEC 61508)
Plant: Heat Treatment
Case Study: Initial stage was for the Hazard and Operability Study (HAZOP) leading on to a detailed Hazard Analysis (HAZAN) followed by a Safety Integrity Level (SIL) assessment and preparation of the Safety Requirements' Specification (SRS).
Full consideration was given to the development of the Safety Life Cycle documentation and IEC 61508 compliance issues. The project concluded with supervision of the installation and commissioning of the plant safety related system.
Contract: Safety Integrity Level (SIL) Determination and Verification
Plant: Petrochemical Plant
Case Study: Retrospective S.I.L. assessment was required on a large section of an existing petrochemical plant.Following the HAZOP report a S.I.L. determination was undertaken for all Safety Instrumented Functions (SIF). to establish a target S.I.L. for each function.Using the very latest software available which included a S.I.L. verification tool for retrospective evolution of existing S.I.F.
Recommendations were made for S.I.L. improvement to achieve target risk reduction factors.
Contract: Independent Safety Assessment
Plant: Crude Oil Refinery
Case Study: This assignment was awarded as part of a €20M control system upgrade for an existing oil refinery in Eastern Europe.
The project commenced with a full HAZOP study with an emphasis on the assessment of the adequacy of existing safe guards and included recommendations for plant improvements.
Prior to entry into the E.U., customer concerns over the requirements of the SEVESCO Directive fully addressed regarding hazard identification and risk reduction requirements.
Contract: Project Health & Safety (CDM)
Plant: Power Station
Case Study: Appointed CDM Co-ordinator under the Construction (Design & Management) Regulations 2007, for this major capital project to ensure compliance with the regulations. The role required that several Design and Site Health & Safety audits were carried out and that the Design Risk Assessments and Health and Safety file was prepared and maintained accurately and timely throughout the project lifecycle. A key role was ensuring good communications between the client, contractors and the client’s insurers.
Contract: Independent Safety Assessment (IEC 61508)
Plant: Power Station
Case Study: Successful contract that called for an Independent Safety Assessment (ISA) to assure compliance with IEC61508 of previously procured safety related equipment. As a result of the ISA, a Hazard Analysis was produced identifying the required Safety Functional requirements for each of the plant skid units and the improvements required to meet the target SIL. The software based Safety Related Shutdown system for the power station balance of plant was designed and built to comply with Safety Integrity Level 2.
A full Safety Integrity Analysis was produced identifying the required SIL (Safety Integrity Level) for the plant. The Safety Related Shutdown system on this plant was designed and built to SIL 2.

Plant: Power Station
Case Study: Appointed CDM Co-ordinator under the Construction (Design and Management) Regulations. This included checking competency of Designers and sub-contractors.
The role required that several design and site health and safety audits were carried out and that the Design Risk Management / Health and Safety files was prepared and maintained accurately and timely throughout the project lifecycle.
